All Applicants must be a resident of the Sylvania school district, be enrolled in the Free and Reduced Lunch Program, and or receiving assistance from JFS. At this time, we are only taking applications for families with minor children in the home.

If you are a Senior Citizen and live in the Sylvania School District, Please call 419-882-8415 to sign up for a holiday food basket.

Due to COVID-19, we will not be distributing “gift baskets”. Participating families will receive a gift card to purchase food and gifts. Please note: Any and all information is subject to verification. If your application is verified and approved, you will receive an email with your date and time to pick up your gift card. You must present a valid ID to receive your gift card.
